Our Responsibilities
- To provide the best available rehabilitation, appropriate to your needs
- To treat you with consideration and dignity
- To give you clear explanations
- To let you see your medical file (on application to the Department of medical
Administration at Liverpool Hospital)
- To treat your medical file and personal information in a confidential manner
- To inform you about other services which may be useful to you
- To respect your cultural and religious beliefs and practices
- To give you and your family information and strategies to help you in your
rehabilitation
- To involve you in the development of your rehabilitation plan
Your Responsibilities
- Once you agree to your rehabilitation plan, please follow our instructions and
guidelines
- If problems arise, please let us know
- You have the right to refuse treatment or withdraw your permission for treatment at any
time, unless it is required by law (e.g. if you are under Guardianship)
- You have the right to seek a second opinion at any time; our doctors or your general
practitioner can help arrange this
- If you want to be treated by another service, either this Unit or your local doctor can
tell you about your options
- Please advise staff about any treatment or therapy you are receiving from anyone else
- Please keep appointments made for you or cancel them if you can't come (we may be able
to fit in someone else)
