If you have been an inpatient in the Brain Injury Rehabilitation Unit at Liverpool
Hospital, appointments will be arranged for you with health professionals on the Community
Outreach Team so that you can continue your rehabilitation while living at home.
If you are new to the service, appointments will be made for you to see a doctor who is a
Rehabilitation Specialist, and a Case Manager if needed. They will arrange for you to see
other team members as necessary.
Case managers are the key contact people for most community clients of the Brain Injury Unit. They coordinate and monitor the implementation of rehabilitation programmes and oversee the progress of rehabilitation through home visits, telephone contact to clients, liaison with therapists and with other service providers. Your case manager will provide you with ongoing support and information and will assist you, your family or important others with accessing services relevant to your and their needs.
Your Case Manager and the rest of the team will help you to work out what your goals are. Everyone works together on a plan to help you achieve your goals. A typical programme might include special assessments, therapy, help from other services and counselling. Regular team meetings are held to check that the plan is working.
You will be discharged when you have achieved your goals or when you no longer need the help of the Community Outreach Team. You can always contact the service for more help or advice at any time after you have been discharged.
