A new service the Cancer Therapy Centre is currently being developed, and offers the ability to book a patient in advance. To assist the Centre in the processing of the patients records, please download, and print off the;

New Patient Registration Form

and have the patient give this to the CTC Reception Staff on arrival. To be able to read this download, you must have Adobe Acrobat Reader installed, if you do not, you can also download and install this file, by simply clicking on the picture below.

This service, is not currently available, however, access to this site will be Password protected, and to obtain a password an Application Form, will need to be completely filled in, and faxed to the Oncology Administration, and marked to the attention of the CTC WebMaster.
