Aged Care Assessment Team

Multidisciplinary assessments are carried out to determine client needs, eligibility for placement
in nursing homes, hostels and Community Aged Care packages (CACP). A nursing home and hostel placement
coordinator assist clients and their families with matters pertaining to placement. The Aged Care Psychiatry
service consists of one community psychogeriatric nurse with assistance from other members of the Aged Care
Assessment team. A consultant Psychogeriatrician attends the Service one day per fortnight. Referrals
are made to Braeside Hospital for inpatient management of selected clients. The ACAT is currently based at Camden Hospital.
