Who Are
We?
The Population Health continuum is a group of health services that focus on:
- Particular population groups eg: young people, aboriginal people
- Health issues covering the whole population eg: drug & alcohol
- Ways of improving health across the population (eg: health promotion) and
- Reducing health inequity between population groups
The services work closely with other agencies to prevent ill health and improve health by:
- Improving access to basic things that ensure good health housing, education, employment
- Providing services in places and in ways that suit the local population
- Encouraging the involvement of people in health care and community activities
- Supporting local residents to take action to improve their own circumstances eg: safety, transport, access to services
- Changing services to make them more appropriate and accessible for a range of population groups
