Long Term Care For admission to the Nursing Home and Hostel, an Application Form and an Assets Assessment Form must be completed. These are obtainable from the Social Worker. The approval 2624 form by the Aged Care Assessment Team (ACAT) - telephone (02) 4640 3568 - is also required. A Residential Care Service Agreement is also to be completed and signed prior to admission. This is also obtainable from the Social Worker. There is usually a waiting period. Respite Care
Dates for Respite Care should be booked in advance with the Social Worker
who will organise an Application Form to be posted for completion. A 2624
form is required and must be completed by the Aged Care Assessment Team (ACAT).
A Respite Care Service Agreement is also to be completed and signed
prior to admission.
