Chronic & Complex Care - Respiratory Liason Nurse

The position is part of the Primary Enhancement Care Program. The service aims to provide quality, safe and efficient management of patients with Asthma and Chronic Obstructive Pulmonary Disease (COPD) in the home environment by Improving networking between hospitals, General Practitioners and Community services to provide a smooth transition between settings. Advantages include;

  • lower length of stay in hospital
  • Prevention of crisis situations & reduction of urgent unplanned admissions to hospital

Services offered

  • Comprehensive respiratory assessment & monitoring vital signs,
  • Monitoring of Peak flow metre , Oxygen saturation & safe use of Home O2.
  • Education to clients/carers regarding signs & symptoms of exacerbation of illness, medication and proper use of devices.
  • Coordinate services between General Practitioner/Specialist and other health professionals.
  • Provision of relevant clinical guidelines to General Practitioner s.
  • Development, implementation & monitoring of care plans in conjunction with the patient's GP and other health workers

Client Group

Must live in Fairfield LGA

Referrals accepted from

  • GP, Specialist, Nursing Home(via GP),ED & hospital wards.

Hours of operation

Monday-Friday
0830 to 1700 hrs.

An Intake officer is available for referrals Monday to Friday from 0900 to 1300 and 1400 to 1600 on 8717-4009.

Eligibility or entry criteria

  • Condition identified as not immediately life threatening
  • Clients > 15 years with diagnosis of Asthma
  • Client informed of the program
